LOST PASSPORT
Re-issue of passport can be applied if an applicant's existing passport is lost, and the applicant have to file a report with the nearest police station. A full-fledged FIR is required if the passport is still valid.
DOCUMENTS REQUIRED FOR LOST PASSPORT :The applicant have to submit Form No. 1 with two different residence proofs, an affidavit and original FIR.
DAMAGED PASSPORT
Re-issue of passport can be applied if an applicant's existing passport is damaged.
DOCUMENTS REQUIRED FOR DAMAGED PASSPORT :The applicant have to submit Form No. 1 with two different residence proofs, affidavit and the damaged passport along with photocopy of it.
